Warranty Replacement

If you bought your mattress at Mattress RX and need to submit a Warranty Replacement Claim,

Warranty Claim Procedure
Step 1

Remove blankets/sheets/protectors etc. from the mattress. Make sure there is adequate lighting in the room and please take pictures of:

1)The Law Tag(s)
(typically found at the head of the mattress). Please take multiple pictures if there are multiple tags or if there is writing on both sides of a tag.

Step 2

Take pictures of the surface of the mattress, taken from the foot of the mattress. Try to capture the full surface as best you can.

Step 3

If the warranty issue is due to breakdown on the surface of the mattress, take pictures measuring the depth of the indentation. Use a string or other straight flat edge, running it side to side on the mattress (not bottom to top) from the high points on the sides across the deepest point of the indentation.

Using a tape measure or ruler, measure the depth of the indentation against the string or flat edge. Take a picture of the measurement from 1 to 2 feet away that clearly shows the string or flat edge and the measurement on the tape measure or ruler.

Take a second picture from the side of the mattress showing how the string or flat edge was strung or laid across the mattress and where the tape measure or ruler was positioned. Take multiple pictures if necessary.

Step 4

Take a close-up of the indentation area from above and 1 to 2 feet away.

Step 5

Remove the mattress and take a picture from the foot of the bed of whatever the mattress was sitting on – foundation (box springs), adjustable base, high-rise frame, or inside of bed furniture. Take a picture at floor level, showing the points of contact of that item with the floor. Take multiple pictures if necessary.

Step 6

If you are using a foundation (box springs) and the foundation is sitting on a frame of any type (“hollywood” frame, bed set, high rise frame), remove the foundation (box springs) and take a picture of the frame from the foot of the bed and another picture at floor level, showing the points of contact with the floor. Take multiple pictures if necessary.

Step 7

If the warranty issue is something other than a surface breakdown, take pictures of the specific problem area along with all pictures listed above except for those pertaining to measurement of a surface breakdown (#3 & #4).
